Well "outpatient" means a patient who's getting treatment at home so I wouldn't assume that. But of course there's no way of knowing exactly what he means by it or if he's just joking or what. There's no evidence in this picture to say one way or the other what kind of care he has been getting or is getting now. It's just a silly photo with a caption that pokes at his situation.
